$25 No Show Fee if the client fails to appear for the appointment.
$15 Cancellation Fee if within 48 hours of the appointment.

1. Client Intake Form
2. Link to a Secure Messaging Portal
Please complete both 1 and 2 as soon as possible so we can create your medical chart and prepare for the appointment.
4. This is a medical appointment, please have available your Medical History, Medication List, and Photo ID so that we can better serve you!
